Wedding Ceremony Contract Price for ceremonies on Friday, Saturday & Sunday $100.00 for ceremonies Monday-Thursday I/we, the undersigned, recognize that I/we are hiring the services of Miguel Carrasco, an ordained wedding minister authorized to perform the rite of marriage in the State of Utah on , 2016, starting at a.m. / p.m. at (venue) in , Utah. I/we hereby hire Miguel Carrasco to write and perform our wedding ceremony on the indicated date above. I/we, agree to pay at time of booking, as a non-refundable retainer. I/we realize that payments on my final balance can be made at any time, however; I/we also understand that the entire balance must be paid no later than the wedding date. Upon signature and receipt of deposit, Miguel Carrasco will reserve the time and date agreed upon, by signing the completed contract. I/we further understand that the contracted fee above is based on the wedding ceremony ending one (1) hour from the stated start time in this contract. Should the wedding start late due to no fault of Miguel Carrasco and not end within an hour of the contracted start time additional fees will be billed at $50.00 per quarter hour. I/we understand that Miguel Carrasco will maintain the original copy of this contract and that I will receive a copy for my files. LEGALITY OF MARRIAGE I/we also understand that it is my/our responsibility to possess a valid marriage license in the state of Utah and that the marriage license must be on the premises when the ceremony is performed. I/we understand that failure to have these legal documents means that Miguel Carrasco cannot legally perform the ceremony and is not obligated to do so, regardless of the inconvenience it may cause. REFUND POLICY I/we acknowledge that the amount that was paid as a retainer is non-refundable. However, if Miguel Carrasco cancels, we will receive a 100% refund on all payments made. I/we also understand that 75% of the work Miguel Carrasco dedicates to our wedding is prior to the actual wedding date, such as the planning, consultations, custom writing of the ceremony script etc. Therefore, I/we understand that if we cancel our wedding after Miguel Carrasco has completed their initial consultation with us (in person, via phone, or via email), and if a draft of our wedding ceremony has already been presented to us, we agree to pay 75% of the total amount of this contract. Both conditions must be met by Miguel Carrasco to be entitled to this payment. For cancellations within 14 days of the wedding date, there is no refund, and 100% of this contract will due to Miguel Carrasco. WEDDING DATE CHANGE If the wedding date is postponed or changed, the following rules will apply. If Miguel Carrasco has the new date available; the date will simply be changed and reserved under the new date. However, if Miguel Carrasco is already obligated on the new date, and is unable to officiate your wedding, then the initial security retainer of Half (1/2) is forfeited and Miguel Carrasco is relieved of all responsibilities set forth in this contract. This process will allow us to address all of her concerns. Current artists, 80's, Motown, Disco, Country, latin and classic rock are just some of the musical genres in our extensive repertoire... Our dinner set is geared to perfectly compliment the most elegant sit-down dining. Everything from instrumental jazz to Sinatra plus Broadway show tunes. We will MC all important moments. We are happy to honor special song requests for first dance, father/daughter dance, etc. Our pricing is for up to 4 hours and include professional quality sound and lights. We also offer a smaller show with fewer performers if desired. Ask about adding a live DJ to cover the pre-show and breaks.

Itinerary : Day 01 : Bangalore – Mysore (140 kms. – 04 hrs.) Upon arrival our representative will greet you at Bangalore Airport/ Railway Station and then proceed to Mysore, the land with numerous domes, towers and battlements. It is sheer luxury in the form of a palace. Reach & check in at the hotel and relax for a while amidst the comforts of your room. Later visit the sprawling Maharaja’s Palace with its outstanding treasures and ivory inlaid doors. Having splendid affair with intricate carvings on the huge palatial doors should not be missed on any account. It is lavishly decorated with stained glasses, mirrors, pillars, paintings and jewels. Also visit St. Philomena’s Church, a beautiful Cathedral reminiscent of Medieval Architecture with stained glass Windows and lofty Towers. Mysore Zoo, one of the oldest zoos in the country was established in 19th century and houses many endangered species. Later drive outside the city to Chamundi Hills to visit the huge monolith Nandi Bull statue, the bull and vehicle of Lord Shiva. Overnight stay at the Hotel/Resort. Day 02 : Mysore - Ooty (164 kms. – 04 hrs.) Enjoy the sumptuous breakfast. Later proceed to Ooty, the land where green mountains and azure sky roll down to embrace quivering beds of crimson roses below. Ooty the place that not only provides natural splendors, but also gives you a taste of its various man-made wonders. Check-in at the hotel and relax for a while amidst the comforts of your room. Later proceed for Ooty sightseeing. The major attractions are Botanical Garden, a stylish garden bordering immense variety of flowering bushes, flowers, plants, fern house with a vast range of ferns and orchids, a clear pool are some of the highlights of this garden. Ooty Lake has an irregular L shape and stretches for more than 2 km in length. The width of the lake varies at regular intervals. "The Dodabetta Peak is located at an altitude of 2,623 meters and is the highest point of Nilgiris hills, Overnight stay at the Hotel/Resort. Day 03 : Ooty Enjoy the sumptuous breakfast. Later proceed for the sightseeing of Coonoor, the place surrounded by the tea plantations. The major attractions are Pasteur Institute, just opposite to the Sim's Park, established in 1907. Sim's Park is the place where individual tree specimens are planted all over the place, giving a special charm to the park. This beautiful park is situated in a deep ravine and has eight major sections. The winding footpaths spreaded all over the park is the most distinctive and lovable feature of this park. Lamb's Rock is 08 kms. from Coonoor and on the way to Dolphin's Nose. It commands a grand view of the Coimbatore Plains. Laws Falls is a beautiful picnic spot, situated at Coonoor-Mettupalayam road at a distance of 07 kms. from Coonoor. The height of the falls is about 180 feets. It is just a paradise for Nature lovers. Overnight stay at the Hotel/Resort. Day 04 : Ooty – Bangalore (297 kms. – 07 hrs.) Enjoy the new sunrise along with the sumptuous breakfast. Later proceed to Bangalore. Reach & Check-in at the hotel and relax for a while amidst the comforts of your room. The major tourist attractions are Vidhana Soudha, a magnificent building of the Neo-Dravidian style which houses the State Legislature. Ulsoor Lake is ideal for rowing around in a boat or to sprawl on the green grass that surrounds the lake. One can sit and relax on the small islands that are on the lake. Lalbagh, is famous for its unique achievement in nurturing the concept of horticulture and aiding the development of horticulture. The Bull Temple, one of the oldest temples in Bangalore situated in Basavanagudi and dedicated to Nandi, the mount of Lord Shiva. It is a sculptural magnum opus which is carved out of a single rock. Cubbon Park - the uppermost attractions are the marble statues of Queen Victoria and Edward VII. Overnight stay at the Hotel/Resort. Day 05 : Bangalore Enjoy the breakfast; later pack your baggages with a promise to return once again to this part of the world....
